Outreach Care Coordinator, Care@Home Program – Full Time

  • Care@Home by Prospect is a home-based primary care program that provides health care services in the comfort and convenience of the patient’s home. It’s like an old-fashioned house-call for patients who are eligible to join. The program provides primary medical care, social support, and help with nutrition and medications.
  • The Outreach Coordinator provides exceptional customer service to Care@Home program participants. This position is responsible for acting as a liaison between program participants, Care@Home field-based providers, participant PCPs, and clinical support staff, including but not limited to, Rounding MDs, NPs, Social Workers, Dietitian, Psychiatrist, mobile laboratory services, etc.
  • The Outreach Coordinator will perform outbound telephone calls to potential program participants to introduce the Care@Home program and will be accountable for weekly scheduling goals.
  • The Outreach Coordinator is responsible for documenting appointments and submitting claims encounters using the EMR Practice Management System, processing authorizations, and other provide back-up support to the Care Coordinator.
  • The Outreach Coordinator will also accept inbound telephone calls from participants returning calls, program staff and other associated parties. Individual will assist program participants via telephone with services necessary to provide high quality, evidence based in-home patient care to complex and high-risk patients.

Job Responsibilities/Duties

  • Outbound and Inbound Telephone Calls to introduce potential participants to the Care@Home program with completion of 100% of call and scheduling goals within defined timeframe. Thorough documentation of each call in Care Management software. Scheduling patient appointments.
  • Front office and care management duties including but not limited to: preparing, reviewing, transmitting claims (including paper and/or electronic claims processing); managing provider schedules and patient appointments; processing referrals and pre-authorizations as required; answering phones; patient and provider support via telephone.

Qualifications

Minimum Education: Bachelor’s Degree preferred. High School Diploma or equivalent required.

Minimum Experience: A minimum of two (2) to three (3) years of care center experience in front office Customer Service, Provider Services or Patient Services. Ability to project excellent customer service, negotiation, and conflict resolution skills via telephone in a fast-pace environment while maintaining a professional demeanor.
