Case Manager I – HIV


This is a Full-time position in Anaheim, CA posted February 23, 2021.

OverviewUnder general supervision by the Case Management Supervisor, the Case Manager will be responsible for functions related to the Psycho Social Department. The Case Manger will work as part of an interdisciplinary team in a primary medical care facility for persons living with HIV/AIDS. The scope of duties include comprehensive psychosocial assessments, implementation of services outlined in service plans and renewals of client’s ADAP eligibility for HIV medications. When necessary, assisting Case Management Supervisor in other administrating tasks.ResponsibilitiesResponsible for intake and assessment of new client’s needs, strengths and resources. Responsible for development of comprehensive Service Plan relevant to the client’s needs. Identifies individuals eligibility for case management services. Provides appropriate referrals, acts as a liaison an advocate for client with other agencies, facilitating access to public assistance programs, other AIDS services, medical needs, mental health services and psychosocial support. Interacts with comprehensive medical and mental health team for the implementation of the comprehensive, quality case plans. Makes referrals to specialty medical care and clinical trials. Monitors and evaluates client’s progress on regular basis. Maintains security and confidentiality of client’s records. Maintains complete, accurate and timely documentation. Keeps abreast of latest medical information regarding HIV/AIDS and services for people affected by HIV/AIDS. Educates clients regarding information and resources relevant to client’s needs. Represents AltaMed HIV Services in the community. Performs all other related duties as assigned.QualificationsPosses a minimum A.A. / B.A. degree in Human Services or a minimum of 3 years of experience working with HIV clients. Minimum of 2 years Case management Experience preferred or equivalent work experience within a related field of Health and Human Services. Knowledge of community resources for public assistance. Case Manager must be certified through the Los Angeles County HIV Case Management Certification process.
